Submit your Immunization Form. If you are a first-time Georgia Southern student, download the immunization form online at http://services.georgiasouthern.edu/health and submit it to Health Services. Students in the following online programs do not have to submit this form: MS Sport Management (effective Summer 2010), MS Kinesiology-Coaching, WebMBA, MED Accomplished Teaching and MED Instructional Technology.
